Responsibilities

Position Description : Responsible to assess, plan, implement, and evaluate the psychosocial services for patients of PrairieCare. The employee provides a comprehensive range of services to patients and their families, including case management and therapeutic services, and seeks to enhance the functioning of patients. This position may work with children, adolescents, and adults and serving patients with co-occurring disorders.

Responsibilities and Accountabilities :

• Have knowledge and understanding of Social Work practices and principles
• Responsible for effective case psychosocial assessment and suicide risk assessment, discharge planning, and leading psychoeducational and / or therapeutic groups
• Responsible for communicating with patients and families to keep them involved in treatment plan decisions
• Coordinates care with outpatient providers upon admission and discharge
• Contributes clinical perspective to the interdisciplinary team based on unique training in the area of family systems and / or social work theory.
• As a therapist, facilitates group, individual, and family therapy sessions
• Will be available to provide coverage for other social workers / therapists when absent or as the need arises
• Communicate clearly and effectively with clinical team members and Directors
• May be assigned supervision responsibilities for those pursuing clinical licensure
• Is oriented and educated in gender, cultural differences, and age appropriate care for the assigned department

Qualifications

Ability to respond appropriately in times of mental health crisis.

Compensation for this role ranges from $29- $34 per hour depending on education and experience.
