- Provide supervision, support, and monitoring for safety concerns.
- Provide crisis prevention / intervention services to the people served.
- Monitor the persons supported for symptoms of decompensation, crisis, and medication side effects, and intervene when necessary.
- Incorporate the CSSP and CSSP Addendum into daily interaction with the persons served.
- Assist the persons supported with problem-solving different aspects of independent living.
- Maintain complete and proper documentation for direct service hours, and fully complete documentation prior to leaving a scheduled shift.
- Participate in scheduled and spontaneous recreational events (while still following the Enriched Living Transportation Policy).
- Maintain confidentiality by complying with HIPAA and MN Data Privacy.
- Adhere to your weekly schedule, arrive on time, etc.
- Attend mandatory staff meetings, and other meetings requested by PC.
- Maintain clear, open, and honest communication with program management.
- Other duties as assigned.
