• Provide supervision, support, and monitoring for safety concerns.
• Provide crisis prevention / intervention services to the people served.
• Monitor the persons supported for symptoms of decompensation, crisis, and medication side effects, and intervene when necessary.
• Incorporate the CSSP and CSSP Addendum into daily interaction with the persons served.
• Assist the persons supported with problem-solving different aspects of independent living.
• Maintain complete and proper documentation for direct service hours, and fully complete documentation prior to leaving a scheduled shift.
• Participate in scheduled and spontaneous recreational events (while still following the Enriched Living Transportation Policy).
• Maintain confidentiality by complying with HIPAA and MN Data Privacy.
• Adhere to your weekly schedule, arrive on time, etc.
• Attend mandatory staff meetings, and other meetings requested by PC.
• Maintain clear, open, and honest communication with program management.
• Other duties as assigned.
