Overview

Under direction to perform, with the responsibility for establishing an education and orientation program for Home Health and assists with coordination and implementation of quality Improvement activities.

Responsible for maintaining Home Health Tech in-service requirements.

Responsibilities

PRINCIPAL DUTIES AND RESPONSIBILITIES(*Essential Functions)

  • Develop short term and long term education objectives for Home Care in collaboration with the Manager.
  • Plans, implements, evaluates in-service, education and orientation programs relevant to the learning needs of all Home Helath Care Staff in a consistent, standardized manner.
  • Maintains current knowledge of Medicare, Medicaid, insurance industry, JCAHO and ISDH regulations for home care services.
  • Provides staff education on updates and changes in federal and state regulatory compliance and CMS conditions of participation in collaboration with the manager.
  • Assists with the development and implementation of orientation process for new employees.
  • Coordinates the development of staff skills competency program and age specific competency program in collaboration with the manager.
  • Maintains clinical library, educational material and resources for staff and patient education.
  • Evaluates staff for patient education skills and provides training as needed.
  • Completes required open and closed chart reviews quarterly and reviews with manager and staff.
  • Utilizes completed chart reviews to formulates educational improvement plans with staff.
  • Develops and implements Performance Improvement projects annually in collaboration with the manager.
  • Reports data to Hospital Performance Improvement Committee (HPIC) monthly.
  • Reviews quarterly Casper Reports from CMS and reports the necessary data to Manager and staff.
  • Reviews Potential Adverse Events report from CMS's Casper Report quarterly.
  • Reviews each event and creates a quarterly report summary for Manager and reports at staff meetings.
  • Provides 12 hours of in-service annually as required for Home Health Techs.
  • Assists in the establishment and revision of Home Care policies and procedures.
  • Responsible in colabaration with the manager for OASIS integrity, education, OASIS errata, SHP OASIS scrubbed reports, review of ADRs and chart audits and reviews.
  • Coordinates Patient Satisfaction Survey mailings, enters responses into database and reports summary quarterly in staff meetings.
  • Responsible for assisting in the development and application of the Home Care EMR : assist with software building, performs testing, is a super-user, acts as a subject matter expert and instructor.
  • Collaborates with Home Health multidisciplinary staff, referral sources, patients and caregivers to promote quality care.
  • Represents Home Care in the community and in hospital activities.
  • Seeks opportunities for self-development through networking, peer collaboration, involvement in professional organizations and related educational offerings.

Qualifications

JOB SPECIFICATIONS(Minimum Requirements)

KNOWLEDGE, SKILLS, AND ABILITIES

  • Must have excellent clinical coordination, interpersonal, verbal and written skills.
  • Must be knowledgeable of home health services and regulations.
  • Must be able to work in a self-directed and collaborative environment.
  • Must exhibit strong organizational, documentation and teaching skills.

EDUCATION

  • Graduate of an acredited college of nursing as a Registered Nurse
  • Associates Nursing Required
  • Bachelors Nursing Preferred
  • Two (2) years clinical experience in a Home Health setting.
  • 2 Home Healthcare Preferred
  • Licensed Registered Nurse
