Connected Health Care is seeking a dedicated and experienced Hospital Social Worker for a full-time position in Great Falls, Montana.

Job Description :
- Patient Support : Provide emotional and social support to patients and their families.
- Care Coordination : Collaborate with medical staff to develop and implement patient care plans.
- Resource Management : Assist patients in accessing community resources and support services.
- Crisis Intervention : Offer crisis intervention and counseling services to patients and families in distress.
- Discharge Planning : Coordinate discharge plans to ensure smooth transitions from hospital to home or other care settings.
- Documentation : Maintain accurate and thorough patient records and documentation.
- Advocacy : Advocate for patient rights and access to necessary services.
Education and Certification Requirements :
- MSW Degree : Must have a Master's degree in Social Work from an accredited graduate school.
- Experience : Minimum of two (2) years of post-graduation experience in a medical setting.
- License : Current clinical license to practice in the State of Montana; LCSW is preferred.
- Skills : Strong knowledge in the principles and practices of social work, counseling, and the ability to work effectively with diverse populations.
- Organization : Must be organized and proficient in completing documentation.
- Communication : Excellent verbal and written communication skills.
- Driver's License : Must have a current Montana Driver's License.
