The primary role of the Registered Peer Recovery Specialist is to help Engage in a wide range of activities, including advocacy, linkage to resources, sharing of experience, community and relationship building, group facilitation, skills building, mentoring, goal setting, etc.
This is a person-centered process which is driven by an individualized treatment plan that functions to maintain the individual’s well-being and prioritize treatment goals and objectives.
The Clinical Case Manager will familiarize him / herself with community resources and draw upon them through linkages and referrals to assist patients in accessing the network of resources that will help maintain their community and promote further growth to maximize functional independence.
The Registered Peer Recovery Specialist completes all processes, procedures, and case management activities directly related to admission, treatment and discharge from the program.
The Registered Peer Recovery Specialist will work collaboratively with the Director of Clinical Services to ensure timely communication with stakeholders and referral sources.
They will ensure all documents are sent to the appropriate team or agency and all follow-up appointments are annotated on the patient’s discharge instructions.
The Registered Peer Recovery Specialist also works with the clinicians, nurses, psychiatrists, patients, families, and community agencies as needed, with the goal of linking patients to appropriate continuing care resources.
The Registered Peer Recovery Specialist acts as a member of a multi-disciplinary treatment team, and serves as an advocate for the patient.
The Registered Peer Recovery Specialist shall function in compliance with all organizational policies and observe ethical and confidentiality standards at all times.
This position will work closely with members of the therapeutic treatment team, external agencies, clients, families, and members of other departments.
