Responsibilities of a Hospice Social Worker - LMSW - PRN :
- Visit patients and families in their homes to assess their needs and provide appropriate resources and support
- Collaborate with the interdisciplinary hospice team to develop and implement individualized care plans
- Provide counseling and emotional support to patients and their families, helping them cope with the challenges of terminal illness
- Connect patients and families with community resources and services to address their needs and improve their quality of life
- Advocate for patients and families to ensure their needs are met and their voices are heard
- Maintain accurate and up-to-date documentation of patient interactions and interventions

Requirements :
A few must-haves for Hospice Medical Social Workers :
- Master's degree in Social Work from an accredited institution
- Current Licensed Master Social Worker (LMSW) certification
- Prior experience in hospice, palliative care, or a related field is preferred
- Strong interpersonal and communication skills
- Ability to work independently and as part of a team
- Compassionate and empathetic approach to working with terminally ill patients and their families
- Valid driver's license and reliable transportation
