Responsibilities of a Hospice Social Worker - LMSW - PRN :

  • Visit patients and families in their homes to assess their needs and provide appropriate resources and support
  • Collaborate with the interdisciplinary hospice team to develop and implement individualized care plans
  • Provide counseling and emotional support to patients and their families, helping them cope with the challenges of terminal illness
  • Connect patients and families with community resources and services to address their needs and improve their quality of life
  • Advocate for patients and families to ensure their needs are met and their voices are heard
  • Maintain accurate and up-to-date documentation of patient interactions and interventions

Requirements :

A few must-haves for Hospice Medical Social Workers :

  • Master's degree in Social Work from an accredited institution
  • Current Licensed Master Social Worker (LMSW) certification
  • Prior experience in hospice, palliative care, or a related field is preferred
  • Strong interpersonal and communication skills
  • Ability to work independently and as part of a team
  • Compassionate and empathetic approach to working with terminally ill patients and their families
  • Valid driver's license and reliable transportation
