Job Description

Under supervision and as members of interdisciplinary teams, Addiction Counselors work with patient in a substance treatment program / service on site at the hospital or in the community.

  • Provide screening and assessing patients in need of addiction or detoxification treatment.
  • Interviewing and providing counseling to patients in need of / seeking addiction treatment or detoxification treatment.
  • Develop biopsychosocial histories of patients to identify and isolate potential SUD and other specific patient problems.
  • Identifying specific psychosocial problems impacting patients' ability to assess treatment, providing assistance in obtaining entitlements.
  • Participating in development of SUD treatment and discharge plans.
  • Conducting supportive groups and community meetings as indicated.
  • Makes decisions and maintains required records regarding appropriate level of care (using patient criteria tools and patient placement criterion systems as needed), to ensure that a patient in need of addiction treatment / services is places in the most clinically appropriate level of care.
  • Upon discharge, referring patients for rehabilitation programs and other treatment modalities.
  • Appropriate professional conduct at all times (see sw departmental professional conduct policy) towards patients, families, visitors, and co-workers, with courtesy, respect, sensitivity to social-cultural and age -related issues, along with awareness of privacy and confidentiality requirements.
  • Complete required documentation in timely fashion, according to assigned area requirements and in professional manner (see documentation policy).
  • Develops and monitors treatment / recovery plans in collaboration with patients and members of the interdisciplinary team, including establishment of short term and long-term goals, objectives, and specific interventions.
  • Participates in the evaluation of treatment progress and monitoring of patient's recovery plans; prepare and maintains confidential case records in relation to treatment goals and objectives.
  • Coordinates and provides care management services and activities that integrate supportive services, agency, professional and / or community resources in a planned framework of action, which address a patient's mental, physical, and / or behavioral needs in order to achieve the patient's established goals.
  • Acts as a liaison and advises staff of patient needs with regard to job training, housing, and education programs; may advise patient on job, education, or substance treatment programs.
  • Conducts outreach activities.

Minimum Qualifications

Any individual working in a NYS Office of Addiction Services and Supports (OASAS)-Certified Treatment Program or newly-hired into the System in the title of Addiction Program Administrator in a NYC Health + Hospitals health care setting must have proof of completion of OASAS "Supporting Recovery with Medications for Addiction Treatment (MAT)" training.

For Assignment to Level I

1. Valid Credentialed Alcoholism and Substance Abuse Counselor (CASAC) - Level 2 Certificate issued by the NYS Office of Addiction Services and Supports (OASAS); or

2. Is otherwise considered a Qualified Health Professional (QHP) based on licensure, certification and / or registration along with appropriate experience and / or training as defined in Section 800.
