The Program Coordinator will play a pivotal role in the planning, development, and implementation of programs tailored to the unique needs of individuals with intellectual and developmental disabilities.
This position ensures compliance with CARF (Commission on Accreditation of Rehabilitation Facilities) standards, ensuring the highest quality of care and services.
ENVIRONMENT :Responsibilities :
- Collaborate with multidisciplinary teams to design, develop, and implement individualized program plans that align with the goals and preferences of clients.
- Monitor program effectiveness and adjust as needed to optimize outcomes. CARF Compliance.
- Stay abreast of CARF standards and ensure ongoing compliance with accreditation requirements.
- Conduct regular self-assessments and internal audits to identify areas for improvement.
- Provide training and ongoing support to program staff, ensuring they have the necessary knowledge and skills to meet CARF standards and deliver high-quality services.
- Establish and maintain a system for continuous quality improvement, including data collection, analysis, and reporting.
- Take proactive measures to address any identified gaps in service quality.
- Advocate for the rights and needs of clients, ensuring they receive the highest standard of care and support.
- Act as a liaison between clients, their families, and the broader community.
- Maintain accurate and up-to-date records in compliance with CARF standards and regulatory requirements.
- Generate comprehensive reports for internal and external stakeholders as needed.
Requirements :
- Foster positive relationships with community partners, local agencies, and other stakeholders to enhance resources and opportunities for clients.
- Complete any additional tasks or projects as assigned by the supervisor or management team.
- Non-standard, Flexible schedule based on program needs.
- Travel as required for training, program, community, and company events.
- High School Diploma or equivalent.
- Minimum of 3 years of experience in program coordination or related role within the IDD field.
- Ability to lift 35 pounds and perform physical tasks associated with the position.
