Position Summary :
The Peer Support Coordinator works with Emergency Department staff to connect with patients experiencing a drug overdose, or patients who present to the Emergency Department with substance misuse concerns, to assist in linking these patients to resources specializing in substance misuse.
Assists the Care Coordination Manager in planning for effective service delivery in accordance with agency policies and contractual agreements.
Takes a lead role in connecting community and training services to the population assigned. The ED Diversion Peer Support Coordinator is a person who has self identified as being in recovery from a substance use disorder, or co occurring disorders of both mental illness and substance use disorder, and is eligible to gain Certified Peer Recovery Specialist status in Tennessee.
Responsibilities
- Provides support for programming and coverage for Covenant Health Emergency Departments.
- Monitors service volumes to ensure compliance with grant requirements.
- Provides direct services at CH facilities to patients who have recently overdosed.
- Refers patients to treatment and recovery support services based on their interests.
- Acquires and maintains community contacts which will serve as client and / or program resources.
- Serves as a resource in the community through attending appropriate meetings.
- Builds and maintains relationships with employees and leadership teams at CH facilities.
- Increases access to treatment and recovery support programs for individuals in the ED who present with substance misuse issues.
- Coordinates with community partners to increase the availability of evidence based recovery programs such as, but not limited to Alcoholics Anonymous and Narcotics Anonymous.
- Coordinates with community provider organizations to improve access to substance misuse treatment facilities.
- Prepares reports in accordance with requirements set forth in MOU between KCHD and Covenant Health
- Other duties as assigned and trained for.
Qualifications
Minimum Education :
None specified; however, must be sufficient to meet the standards for achievement of the below indicated certification as required by the issuing authority.
Minimum Experience :
Lived experience with substance misuse and 2 years successful recovery.
Licensure Requirement :
Certified Peer Recover Specialist, or enrolled.
