Job Summary :
Coordinates patient care needs by facilitating patient placement in the appropriate nursing unit utilizing pre-established criteria and consulting with members of the interdisciplinary team.
Receives and processes requests for assistance with acute patient transfers and / or admissions of patients from medical facilities.
Job Requirements :
Education and Work Experience :
- Bachelor's Degree in Nursing (BSN) : Preferred
- Experience as a lead or charge nurse or with placement or care of critically ill patients : Preferred
- Experience working with electronic medical records (EMR) : Preferred
Licenses / Certifications :
- Registered Nurse (RN) licensure in the state of practice : Required
- Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS OR HS-BLS OR RQIBLS) certification : Preferred
Essential Functions :
Performs telephonic patient triage, discharge follow-up with physicians and patients and disease management programs. Coordinates with other clinics and hospitals, assisting with physician consultation, transfer and coordination of patient ground / air transport.
Collaborates frequently with Emergency Room (ER), and Post-Anesthesia Care Unit (PACU) and bed boards to keep abreast of possible admissions / transfers.
Collaborates with the Bed Assignment / Staffing Group, Patient Placement Coordinator or Administrative Supervisor per shift to determine census and acuity.
Collaborates to resolve bed availability challenges. Receives and obtains the admission orders for admissions and ensures that orders are sent to the appropriate nursing unit
Documents coordinator and patient interactions in electronic or hardcopy chart in a manner consistent with hospital standards.
Creates reports from patient data such as volumes and outcomes.
Performs other job-related duties as assigned.
