Responsibilities :
- Provide intermittent skilled nursing services in a face-to-face capacity to patients at their residence or qualifying facilities
- Assess and evaluate patient needs and collaborate with patients to set goals
- Communicate patient progress with other disciplines and report to clinical supervisor and referring physician
- Develop and update patient care plans based on identified needs and goals
- Perform skilled nursing visits according to schedule and submit required documentation
- Collaborate with the continuum of care team and participate in the discharge planning process
- Act as a preceptor in the orientation of new nursing staff
- Participate in staff meetings, team conferences, and educational in-services per agency requirements
- Follow agency policies and procedures
- Participate in Process Improvement (PI) program by assisting with data collection and serving on PI team
- Responsible for ensuring the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and providing Age-Friendly Care
- Other duties as assigned
Qualifications :
Required Knowledge, Skills and Experience :
- Current unencumbered State professional Nurse License
- Valid driver's license and good driving record
- Ability to make sound clinical judgment, assess patient needs, and formulate care plans
- Proficient clinical skills and excellent verbal and written communication skills
- Proficiency in personal computer use
Additional Requirements for Florida :
Active CPR Certification
Preferred Skills and Experience :
One year of experience as a home care professional nurse and competency in performing home care comprehensive assessment
