Our Outpatient team has a fantastic opportunity for a Clinical Case Manager.
As a Clinical Case Manager, you will work as part of a team to provide outpatient mental health services and community support.
You will be responsible for writing treatment plans and reviews in a collaborative format and work with clients to identify goals and methods for success.
Our Case Managers work with a case load of adults with severe and persistent mental illness providing treatment based on client need that includes identifying and using community resources, liaison with other member treatment providers, referral to other appropriate community programs, and life skills training.

SCHEDULE
The shift for this schedule would be Monday-Friday 9 : 00AM-4 : 00PM.
RESPONSIBILITIES
- Advocate for member needs which include but are not limited to medical appointments, welfare & vocational programs, and legal systems
- Provide comprehensive therapeutic community support to an assigned caseload.
- Provide individual treatment sessions when appropriate to member needs and per prescribed treatment planning
- Outreach to member homes to assess living situation, stability, and provide treatment interventions when necessary
- Facilitate assigned group therapy sessions and specialized member events i.e. field trip, Christmas party, etc
- Assist in discharge planning and written summaries of treatment as needed for member referral and releases of information
- Evaluate treatment effectiveness with agency medical / clinical team for voluntary and involuntary clients including revision of all treatment documentation as required by the WAC and per supervision direction
- Writes and keeps progress notes per agency guidelines
- Attend scheduled meetings and conferences, including at least two hours per week (per FTE) with Clinical Director and / or Psychiatrist
- Provide immediate crisis intervention and resolution as needed
- Visually supervise medication distribution as necessary
- Maintain member and staff confidentiality at all times
- Other duties as assigned
QUALIFICATIONS
- Bachelor’s degree in social science or other related field
- Minimum 6 months of clinical experience working with severe mental health and / or SUD individuals
- Current AAC registration and ability to apply for one within 30 days of hire
- Current CPR / First Aid certification or ability to obtain one within 30 days of hire
- Have legible handwriting
- Ability to do simple mathematics
- Ability to read and comprehend agency / member charts
- Must hold a valid WA State driver’s license for 1 year with clean driving record to use agency vehicles.
- Prior experience with mental health population
- Ability to perform duties outlined above within reasonable accommodation
