JOB SUMMARY :
The Licensed Practical Nurse (LPN) coordinates patient care following all established standards of practice within the State and Federal guidelines.
Provides staff mentorship. Serves as a resource to both staff and management in problem identification and resolution. A conscientious and organized person who schedules all surgical cases.
When needed, pre-certifies patients for surgery. Maintains communication with OR scheduling staff, patients, and providers.
MAJOR ACCOUNTABILITIES / CRITICAL RESPONSIBILITIES :
Reviews schedules and organizes patient low; greet patients, documents patients concerns, questions etc accompanies patient to exam / procedure room and prepares patients for physical exam.
Initial patient interview; reviewing patient medical profile, and documents all required charting in EMR / manual charting as appropriate.
Obtains and accurately documents patient vital signs, weight; reports any abnormalities to physician. Assists with specimen collection and testing as necessary.
Documents pertinent information.
- Assists patients as necessary . walking, transferring, dressing etc.
- Provides minor treatment as directed by physician, assists physicians with minor procedures.
- Provides education as indicated after evaluating the learning needs of the patient and or family.
- As required, schedules patient testing and pre certifies if insurances requires.
- Responds to telephone messages (patient, physician offices etc )
- End of month checks for expired supplies and medication.
- Stocks rooms / maintains inventory or exam / procedure rooms.
- Orders supplies as needed.
- Exam / procedure room & equipment cleaned after each patient.
- Prepares exam / procedure room for next patient & physician needs.
- Notifies patients of test results (per physician instruction) and follow up.
- Attends scheduled staff meetings, in service education opportunities and participates in continuing education programs.
Formulates and maintains effective working relationships with employees and other managers of the health care team.
- Schedule surgery for all Patients as necessary. Schedule post-operative appointments and enter into the physician’s schedule.
- Provide the patient with pre-operative instructions and answer any outstanding questions or concerns the patient may have regard the procedure as appropriate.
- Write pre-operative request and fax to PCP or specialist as appropriate requested physician.
- Receive & Review pre-operative testing & history and physical from requested PCP’s or specialist as appropriate physician’s office and submit to pre-admissions department for review.
- Call insurance company for verification of CPT / ICD 10 codes for prior authorization.
- Document prior authorization in patients’ medical record.
- Provide self-pay estimates for surgeries and in-office procedures to patients as necessary.
- Coordinate surgeries with other physician offices for shared surgeries.
- Call PCP or specialist as appropriate requested physician office for patients to book the pre-operative clearance visit when possible.
- Call PCP’s specialist as appropriate requested physician office to follow up on medical clearance and testing for surgeon’s review if not received in specified timeframe according to current protocol.
- Receive and answer telephone calls from patients, regarding questions about surgery times and / or cancellations.
- Respects and protects the patient’s rights to confidentiality and privacy and discloses information only for the professional purposes which are in the patient’s best interests with full consideration of their legal rights.
Displays a positive attitude when interacting with physician’s office staff, physicians, and fellow employees of the office.
- Participates in and supports performance improvement and risk management activities as requested.
- Performs other related duties as assigned or requested in order to maintain a high level of service. Completes required continuous training and education, including practice specific requirements.
Demonstrates professional work behavior by following Service Standards. Complies with departmental organizational policies and procedures and adheres to external agency requirements.
QUALIFICATIONS / REQUIREMENTS :
- Graduate of an accredited school of nursing.
- Valid license as Licensed Practical Nurse in CT.
- BLS Certification.
- Excellent customer service / communication skills.
- Knowledge of medical terminology.
- Basic computer skills are necessary.
- Minimum of 2 years previous experience in a similar position preferred.
- Must be able to read, write and speak the English language.
