We are looking for a Licensed Social Worker - LSW to join our dynamic team.
Pay $59K to $68K Annually
Licensed Social Worker - LSW Qualifications :
Minimum of one year of experience in Long-Term Care.
BSW, with a current MA Social Work License.
Read, write, and speak English fluently.
Licensed Social Worker - LSW Job Summary :
Responsible for assisting and empowering residents to maintain and achieve their highest level of practicable functioning.
Participating with the interdisciplinary (IDT) team in developing, implementing, and revising resident / patient care plans.
Address the ongoing psychosocial needs of residents / patients by providing a range of social services such as individual and family support or dementia family support groups or other group supports around adjustment or loss.
When RN CM not available, set up discharge plan for skilled patient for a successful transition back to the community (referrals made to VNA, hospice, set up DME and other services as needed in the community).
Scheduling an interdisciplinary family / resident meeting upon admission to review treatment goals and progress.
