Director, Community Partnerships and Programs

A strategic leadership position with oversight of the daily operations of a geographically defined Gilchrist neighborhood (e.

g. counseling and support, elder medical care and hospice) while ensuring a vision and strategy for future growth. Daily operations would include but not be limited to patient access, operating budget, contractual compliance, knowledge of insurance referrals and preauthorization requirements, health information management, patient care transitions, environmental safety, regulatory compliance, patient safety, quality initiatives, and patient and family experience.

This position is responsible for developing and enhancing relationships with health systems, residential care facilities, and community leaders and organizations with a goal of increasing visibility and community affiliations and partnerships.

Supervises Neighborhood Leaders.

Bachelor's Degree in Nursing or Health Care Administration or another related field.

Master's Degree Preferred

Experience :

10 plus years of progressive leadership experience in a health care related field, preferably in serious illness or end of life care.

Demonstrated experience in network collaborations
