- instruct families on this benefitCollect contact information for patient and close acquaintances who can reach themAdminister vaccinations safely and properly as assignedAccurately document / communicate interventions and measurements in patient clinic recordCheck clinic records to update contact information as neededCollaborate with UTMB and non-UTMB (county or school) clinics to coordinate vaccination appointments to ensure completion of the vaccine seriesSet up phone and text reminders for the patient / patient family in their preferred language for all appointmentsBuild Hospital Account Record (HAR) for every follow up appointment, checking Medicaid eligibility and insurance statusSchedule / reschedule vaccination appointments as neededPersonally call patient / patient family to reschedule appointments;
send letters when patients cannot be reachedMaintain accurate records of all patients approached and number of vaccines receivedCollect high-quality data using patient enrollment forms and EMR systems.
Accurately enter and validate data in an electronic database using all data collected at UTMB, and from ImmTrac, and update on a daily basis.
