may include travel to clinics in Galveston, Clear Lake, Houston and surrounding areas.Confirm patient vaccination status (with patient family, ImmTrac, EMR, etc.

  • determine eligibility, and obtain informed consentCounsel patients about the importance and safety of the HPV vaccination (weekend and holidays may be included)Determine eligibility for the federal Vaccines for Children Program and Medicaid;
  • instruct families on this benefitCollect contact information for patient and close acquaintances who can reach themAdminister vaccinations safely and properly as assignedAccurately document / communicate interventions and measurements in patient clinic recordCheck clinic records to update contact information as neededCollaborate with UTMB and non-UTMB (county or school) clinics to coordinate vaccination appointments to ensure completion of the vaccine seriesSet up phone and text reminders for the patient / patient family in their preferred language for all appointmentsBuild Hospital Account Record (HAR) for every follow up appointment, checking Medicaid eligibility and insurance statusSchedule / reschedule vaccination appointments as neededPersonally call patient / patient family to reschedule appointments;

send letters when patients cannot be reachedMaintain accurate records of all patients approached and number of vaccines receivedCollect high-quality data using patient enrollment forms and EMR systems.

Accurately enter and validate data in an electronic database using all data collected at UTMB, and from ImmTrac, and update on a daily basis.
