Job Description

The Habilitation and Support Coordinator ensures compliance with Individual Support Plan (ISP) implementation. The Habilitation and Support Coordinator facilitates communication between site managers, Service Coordinators, and interdisciplinary teams.

Essential Duties & Responsibilities

  • Support with preparation for the Annual ISP meeting, attending when possible, and request additional meetings for changes in service.
  • Monitor and report on the implementation of staff objectives and habilitative goals.
  • Ensure all programs are written in compliance with state regulations and in accordance with the ISP.
  • Complete all end-of-month goal reviews and analyze outcomes for progress.
  • Ensure that Functional Behavior Assessment and Behavior Support Plan are compatible.
  • Review the documentation for completeness and assist in resolving any issues identified.
  • Check in with site managers and schedule preparation for upcoming ISPs, meetings, evaluations, etc.
  • Work with the interdisciplinary team regarding circumstances which may require an increase in supports.
  • Work with behavioral team and medical team to complete and submit exception requests to DHHS State Review Team.
  • Monitor exception recommendations, request and submit renewals when needed.
  • Participate in behavioral team meetings; contribute to and participate in DHHS Risk Review meetings.
  • Other duties as assigned.

Education & Experience

  • At least two years of experience working with people with developmental disabilities
  • Valid driver's license
  • Current automobile insurance
  • Ability to pass registry and background checks

  • Proven skills in communication, observation, problem solving, and time management
  • Excellent time management skills and the ability to juggle multiple responsibilities while communicating honestly about your timelines, challenges, and questions
  • Willingness to learn new skills and techniques

    • Physical Requirements

  • Ability to perform repetitive tasks for extended periods of time to include but not limited to bending, stooping, sitting, and reaching
  • Ability to lift up to 25 pounds as needed
