POSITION SUMMARY :

We are seeking a Speech Pathologist to :

• Join our world-class team of doctors, nurses and other health care professionals in providing everything from preventive health care to highly specialized programs.
• Provide quality care in a compassionate patient- and family- centered environment.
• Plan, evaluate and administer Speech Therapy services and care to patients in the home as prescribed by referring physician on an intermittent basis in accordance with Agency policies.
• Coordinate family involvement in assessment and treatment of the patient.
• Participate in continuous quality improvement activities and educational experiences in support of departmental philosophy and objectives, as well as Health System initiatives.

EDUCATION / TRAINING :

Master's Degree in Speech Language pathology.

LICENSURE / CERTIFICATION :

• Clinical competency Bi-annual and CPR certification.
• Current license from the State of Illinois or in the process of acquiring licensure.
• An applicant for employment as a physical, occupational, or speech therapist must present acceptable evidence of licensure or authorization to test at the time of employment.
• An applicant who fails to provide this documentation will not be hired.
• A new employee will not be allowed to work as a therapist or assistant until evidence of licensure eligibility is provided.
• Valid State driver's license and proof of liability insurance.

EXPERIENCE / SKILLS / ABILITIES :

• Home Care experience preferred.
• Must be skilled in a broad range of modalities with clear understanding of indications and contradictions for each.
• Ability to effectively communicate orally and in writing in a caring, considerate and concerned manner.
• Knowledge of TJC, IDPH and CMS rules and regulations.
• Willingness to accept responsibility.
• Ability to direct technical, assistant staff and students and organize work assignments.
• Leadership skills essential to the position.
• Successful completion of annual clinical, age and job specific competencies and skill verification tools required.
