PRIMARY DUTIES AND RESPONSIBILITIES :

  • Conduct individualized service assessments for each participant to identify their key service needs.
  • Provide information about the services available (at CKSC as well as through a broad network of community-based providers) to each participant in accordance with their needs and preferences.
  • Provide linkage and referrals to any services that participants wish to receive, coordinating with other providers as needed.
  • Assist with accessing necessary services, including support with referrals, such as completing applications and forms, contacting agencies on behalf of the participant, ensuring that participant questions are answered, and otherwise supporting participants to successfully access services to which they have been referred.
  • Conduct follow-up for referrals to ensure that participants were able to access and receive services, and that the services received were the correct and appropriate ones to meet participant needs.
  • Conduct regular outreach to welcome new participants to the ADRC at least three times per quarter, in close coordination with the Citywide ADRC Coordinator and other ADRC sites, which will include posting flyers, making presentations at emergency shelters, permanent supportive housing sites, and adult education and vocational program sites;
  • coordinating with other ADRC sites; attending senior resource fairs and community or provider events that target seniors;

participating in Project Homeless Connect, as well as any other pertinent DAS Community Partnership meetings such as Latino partnership, African American partnership, and Asian Pacific Islander partnership.

Attends community meetings as needed.

Qualifications

REQUIREMENTS :

Bachelor’s degree in health, social work, counseling, psychology, gerontology, or other related fields, and a minimum of two years of direct Information & Referral experience.

Fluent in Tagalog.

Must have experience working with elderly and young adults with disabilities. Must have proven organizational skills, computer literacy and a genuine commitment to the mission and interests of Episcopal Community Services and San Francisco’s seniors and disabled population.

OTHER :

Must secure annual TB test. Must be finger imaged . All offers are contingent on successful passing this background check.

Will honor and abide by rules of confidentiality. Office space will be shared and will function as a group room when not in use as an office.
