We are looking for a highly motivated and energetic Medical Assistant / Care Coordinator to join our team as a Chronic Care Management (CCM) Onboarding Specialist!
In the role of a Chronic Care Management (CCM) Onboarding Specialist, you will serve as a key link between our patients and the essential services we provide.
This role includes a strong focus on improving patient outcomes through effective communication, coordination of care, and compliance with healthcare regulations.
Primary Responsibilities :
1. Patient Outreach and Enrollment : Actively promote our Chronic Care Management program to qualifying patients, clearly articulating the program benefits and securing their consent for enrollment.
2. Patient Engagement : Maintain proactive communication with patients to address their queries regarding CCM services, and document all interactions diligently.
3. Care Coordination : Execute onboarding calls, follow-up calls, medication checks with pharmacies, and liaison with healthcare providers to obtain relevant medical records.
4. Healthcare Systems Navigation : Proficiently utilize various Electronic Medical Records (EMR) systems and adapt to the specific practices of various doctor offices.
5. Patient Advocacy : Manage refill requests and other patient needs during and after onboarding calls.
6. Compliance and Quality Assurance : Strict adherence to Health Insurance Portability and Accountability Act (HIPAA) guidelines and regulations.
Qualifications and Skills :
- Excellent verbal and written communication skills
- Chronic Care Management experience
- Familiarity with EMR systems
- Strong attention to detail and organizational skills
- Proficiency in time management and ability to multitask
- A solid understanding of HIPAA guidelines and regulations

Work Location : Enroll patients face to face in a Medical Practice across multiple Locations in Metro Detroit, MI . Mileage will be reimbursed.
