LPN Job Summary :
A Licensed Practical Nurse performs selected nursing care for Patients in Care of Sick, and / or Hospice Programs as delegated by the PCN whom she / he works in a nursing team relationship, under general supervision of the Clinical Supervisor.
LPN Essential Responsibilities :
- Collaborates with PCN to review Plan of Care and provide nursing care to patients in their homes on a family centered basis while recording / maintaining complete, accurate patient record on Portable Electronic Device and / or via paper record.
- Makes patient assessment, reports and documents appropriately as delegated by the registered nurse within their scope of practice.
- Works under direction of PCN in orientating and delegating responsibility to the Home Health Aides within Regulatory Guidelines as per agency policy.
LPN Education Qualifications :
- LPN Degree required.
- Current CT license required.
LPN Experience Qualifications :
- Minimum of four (4) years of nursing practice, two (2) years of which must be recent Med-Surg, acute care, or critical care experience as a Licensed Practical Nurse preferred.
- Valid Connecticut Driver's License and proof of current insurance coverage required
