Care Partners, a small nonprofit hospice, is seeking a full time Community Liaison to join their team. The primary focus for the role is to leverage opportunities with existing partners, expanding the partner base through business development activities, and consistently delivering superior customer service.
In this role you’ll have the opportunity to play a key role in promoting Care Partners services and values to strengthen and grow admission referrals.
Care Partners is a 501(c)(3) nonprofit that offers serious illness care and support to individuals and families in greater Portland.
Our goal at Care Partners is to maximize the comfort and health of patients and families consistent with hospice philosophy along with the organization’s policies and procedures.
Responsibilities include :
- Contribute to and execute a territory plan to grow admissions and referrals in Long Term Care communities, clinics and existing referral sources.
- Assist in creating, developing and maintaining a marketing plan for the organization.
- Generate business by establishing new and maintaining existing relationships with potential referral sources, making regular visits to sites that don’t have call restrictions, presenting programs, and meeting annual goals as established.
- Contribute to the design and implementation of marketing and social media campaigns including collection of patient / family stories.
- Pro-actively seek out business development sources through networking, relationship building and data mining.
- Maintain contact with and develop deep understanding of existing client needs to identify and anticipate new opportunities.
- Assist with the collection and analysis of admission and utilization data by referral source.
- Utilize client relationship management software to maintain contact database, identify leads and track activities.
- Conduct individual / group presentations in long term care communities.
- Respond to partner inquiries and requests in a timely and professional manner.
- Consult with prospective and existing partners to resolve problems and to provide ongoing support.
- Participate in community and organizational programs and promote positive public relations with patients, family members and healthcare providers.
- Communicate the organization’s value proposition relative to the needs of the local community and target patient populations.
- Engage and educate community partners through in-services alongside clinical team members.
Qualifications / Experience
- Requires a bachelor’s degree with a focus on business administration, marketing or a related field or equivalent in work experience and / or training.
- Must have a minimum of one year of professional outreach experience and a proven record of success in growing business in a competitive environment, preferably in the hospice and / or health care industry in a state without certificate of need.
- Excellent verbal and written communication skills including work experience that demonstrates skills in relationship building and successful influence in the course of daily job activities.
- Ability to develop customer and co-worker relationships effectively and make decisions in fast-paced, patient-centered operation using exceptional customer service, critical thinking and problem-solving skills.
- Effective interaction with patients / families, referral sources, and the community in potentially difficult end-of-life situations.
- Proven professional presentation, public speaking, written and overall interpersonal communication skills.
- Requires the ability to work independently and as part of a team and be reliable in completing assignments and activities assigned.
- Proficiency in Microsoft Office, social media and client relationship management software.
- Must have a valid’ driver’s license, acceptable driving record, and have access to reliable transportation for local business travel.
Schedule : Monday-Friday, 8AM to 5PM. Work may be performed through a combination of in-office, remote work, and field work arrangement to be determined at time of job offer.
