Salary : 20-24 dollars per hour
At David’s Loft Clinical Programs, we owe our success to people and processes. A Case Manager is a supportive force behind both, and we are currently seeking someone exceptional to take the helm.
The ideal candidate will engage in a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote quality, cost-effective outcomes.
The candidate should also have experience working in an office environment, performing administrative tasks, and providing support to coworkers.
An ability to multitask, manage complex schedules, and meet changing deadlines is essential to the position.
Status : Non-Exempt
Responsibilities :
- Assess client’s or family’s needs and eligibility for services
- Works with clients, family and appropriate community agencies to coordinate and facilitate services for the client
- Participate in planning treatment programs and in dispositional and discharge plans
- Provides individual and group counseling to client and possibly families
- Maintains family contact regarding client adjustments
- Attends and testifies at court proceedings
- Attends staff, team and department meetings
- Attend in-service, staffing and other meetings with supervisor’s approval. May be appointed to committees
- Actively participate in team review of record and cases
- Participate in agency and community planning and education
- Participates in disaster evacuations and fire drills when circumstance warrant
- Provide selected agency services to identified caseload; refers clients and families to other service as appropriate
- Develops and maintain clients case record, plans and reports
- Other duties that may be assigned to drive our organization's mission, vision, and values
Requirements and Qualifications :
- Knowledge of office management systems and procedures
- Excellent time management skills and ability to multitask and prioritize work
- Attention to detail and problem-solving skills
- Excellent written and verbal communication skills
- Strong organizational and planning skills
- Proficiency in MS Office.
- Bachelor’s Degree in health-related field with licensure as appropriate
- Three years of related experience in a health care setting and Community, case management, and / or outreach experience
- Certified Peer Recovery Specialist highly desired or willing to obtain within first year of employment
Physical Capacity :
Must be able to sit and stand for 20 minutes and lift at least 10 lbs.
