Job Purpose :
The Treatment Foster Care Level 2+ Coordinator is responsible for filtering, coordinating, and facilitating meetings around licensed or soon to be licensed foster homes that are eligible to move from Level 2 to Level 2+ with additional support.
The Level 2+ Coordinator will work closely with case management and licensing to ensure the file is organized and has all updated information, organize and facilitate staffing’s and to make the decision if the home will be accepted into the Level 2+ program.
QUALIFICATIONS :
Bachelor’s degree in social work or a human related service field required.
Five (5) years of experience working with children and families in social
services in any; family, group and individual service delivery experience preferred.
- Two (2) years of experience working in a social work-related with treatment foster care case management and / or foster licensing experience preferred.
- Knowledge of community resources.
- Demonstrated ability to achieve outcomes and work successfully with internal and external stakeholders.
- Possesses stability and personal attributes necessary to work with other departments and staff.
- Ability to organize time effectively, maximizes use of available resources, successfully balances the competing demands of multiple projects by setting priorities and communication and meeting deadlines, asks for help when necessary.
- Demonstrates adaptability, recovers quickly when progress is stalled by obstacles; responds with flexibility and resilience when faces with multiple demands, shifting priorities and rapid change.
- Takes initiative, acts quickly and independently when the situation demands it; sees a need and takes appropriate action without being prompted or reminded;
demonstrates ability to effectively deal with crisis situations.
- Exhibits critical thinking skills when faced with an issue.
- Excellent communication skills.
- Proficient in the usage of computer software; demonstrate an understanding of computer file systems and computer software packages, e.
g., Microsoft Office, Excel, Outlook, Teams, ADP, etc.
- Must be honest, dependable and able to meet deadlines; Self-motivated and able to work independently.
- Physical exam, drug screen, motor vehicle report, and background checks are required for this position.
JOB RESPONSIBILITIES :
Coordination and Direct Service
Interprets, applies and enforces requirements of DCF Administrative Code 12, 54 and 56; Wisconsin State Statute Chapter 48;
Indian Child Welfare Act, COA standards and Agency policies and accreditation standards.
- Identifies, maintains and refreshes a list of potential Level 2+ caregivers using available records and resources.
- Completes initial assessment of potential Level 2+ caregivers utilizing a structured process.
- Attends home visits to discuss the Level 2+ process to caregivers.
- Meets with families to complete necessary documents to enter into Level 2+.
- Provides consultation and support to caregivers to identify specific training opportunities and areas of foster parent development as needed to support specific foster parent / client needs in a timely manner.
- Reviews documentation and files for quality of transition.
- Monitors and tracks outcomes of families that participate in Level 2+ program.
- Supports caregivers that do not transition or desire to leave Level 2+ programming with reestablishment into the Level 2 program as appropriate.
- Provides education to existing caregivers and external stakeholders on the benefits of Level 2+ and how to move between levels of care to meet the needs of children and caregivers.
- Partners with case management and licensing to discuss benefits of enrolling in the Level 2+ program.
- Collaborates with case management and licensing to best meet the needs of the families we serve.
Program Assistance
- Coordinates and facilitates staffing’s and meetings.
- Follows protocol regarding client communications while maintaining confidentiality of all departments / client communications.
- Participates in program activities including foster parent support groups, appreciation events and training.
- Develops, reviews and revises protocols and processes regarding staffing and transitions.
- Works closely with other counties developing relationship and education around Level 2+ support
- Collaborates regularly with marketing to maintain current materials for Level 2+ recruitment which includes connecting with program leaders across the agency to share information on how Level 2+ can best support caregivers and children.
- Documentation
- Documents all applicable licensing information and case notes into the Agency’s licensing database, Cobris, and / or other required systems within 72 hours.
