Physical Therapist DPT - Home Health
We are seeking a Physical Therapist for a full-time and 100% Home Health focused opportunity in Longmont or Longmont / Thornton are, CO!

Compensation Package :
- $57 to $65 Per Regular Visit
- $80 to $95 Per Non Medicare Visit
- $95 to $115 Per Medicare Visit
- $80 to $95 Per Evaluation Visit

Requirements :
- Clear and Active PT License in Colorado
- New Graduates Considered
- Current and Active Driver’s License
- Home Health Experience Preferred - Training Offered
Responsibilities :
- Monday through Friday 8 AM to 5 PM
- 1 Saturday or Sunday Every 4 to 6 Weeks for Additional Compensation OR 1 Day Off
- Working with Geriatric Patients
- Using Matrix Care EMR and OASIS Documentation - Training Offered on Both
- Seeing 5 Geriatric Patients Per Day in Their Home - Routes Are Coordinated Depending on Your Home Location
- 1 to 2 Weeks of Onboarding / Orientation
- Comply with ethical and professional standards of conduct as set forth by the American Physical Therapy Association.
- Assess each patient and developing an appropriate treatment plan with measurable goals and time frames.
- Demonstrate treatment skills that are rooted in evidence-based practice guidelines while balancing both patient and provider expectations and needs
