SUMMARY OF POSITION :
The Care Coordinator is an integral member of the interdisciplinary team, with expertise in the area of assessment, discharge planning and the other psychosocial environmental aspects of patient and family dysfunction.
Demonstrates competency with interventions and clinical judgment based on the population and age specific needs. Utilizes clinical expertise to provide group, family, and individual therapy through various modalities to aid in successful transition readiness.
MINIMUM QUALIFICATION(S) :
- Master's degree in psychosocial scienceÂ
- Two (2) years of experience in the mental health fieldÂ
- Act 31 training completion required within 45 days of hire.Â
PREFERRED QUALIFICATION(S) :
- Master's degree in Social Work
- Social Work Licensure
