A company is looking for an I / DD In-Reach & Transition Coordinator to support individuals with Intellectual Developmental Disabilities in transitioning to community-based services.
Key ResponsibilitiesIdentify consumers / enrollees in State Developmental Centers and ICFs for potential community placementFacilitate the transition process by coordinating housing options and support services for individualsDocument progress and maintain clear communication with stakeholders involved in the transition planningRequired Qualifications, Training, and EducationBachelor's degree in public service administration, psychology, sociology, social work, or a related field with four years of relevant experience, or a Master's degree with two years of supervised clinical experienceExperience in case management, assessment and referral, supportive counseling, and treatment planningDirect experience working with children, adolescents, and adults with mental health and intellectual / developmental disabilitiesKnowledge of medical, behavioral, and psychosocial issues relevant to the population servedAbility to work effectively in a multi-disciplinary team environment
