Job Summary :

Implements effective strategies for linking uninsured and underinsured clients to primary care, increases access to appropriate medical care and community-based services, assists individuals in improving health status, and promotes behavior change in using Morton health care service s.

Essential Duties & Responsibilities :

  • Provide focused care management by linking clients to the most appropriate venue for entry into the healthcare system, including primary health care providers, medical home, and specialty care services for chronic disease and lifestyle management.
  • Coordinate care with physicians, case management, and other community partner resources to provide coordinated care for patients.
  • Help patients set health goals and health management plans, coordinate patient transportation as needed to scheduled appointments, and monitor assigned patients’ progress and outcomes.
  • Increase patient understanding of preventative and maintenance health care by providing education and resource information.
  • Act as a patient advocate by providing social support and bridging gaps in client’s service by conducting patient health education and set follow-up appointments.
  • Notify clinical team of changes in behavior, nutrition, exercise, substance abuse, medication compliance, and other issues related to the patient plan of care.

Maintain complete and accurate documentation of all activities following federal, state, and Morton policies, procedures, and regulations.

Job Qualifications :

  • High School Diploma or GED required.
  • Healthcare, social work, or public health background preferred but not required.
  • Understanding of the healthcare delivery process is required.
  • Excellent interpersonal communication skills are required.
  • Broad knowledge of community resources and services is required.
  • Bilingual (English / Spanish / Burmese) preferred but not required.

This position has been deemed a Safety-Sensitive position; therefore, Morton reserves the right to refuse to hire an applicant or may terminate an employee in a Safety-Sensitive Position who tests positive for medical marijuana even if they have a valid medical marijuana license.
