Position Summary :
The Support Coordinator in the Support Services for Adults (SSA) Program is responsible for the coordination of all aspects of the individual planning process.
The plan is designed to meet the individual needs of each person served. This includes communicating, building a rapport, and relating well to all persons served, families, team members, and other stake holders.
Pay Rate : $23.27 / Hr
Full-Time 40 Hrs. Mon-Fri, Day Shift
Immediate Supervisor :
Support Services for Adults Center Manager
Primary Responsibilities and Duties :
- Ensures the safety and welfare of all persons served, families and visitors.
- Provides direct support as needed or assigned.
- Coordinates Individual Plans for SSA persons served.
- Responsible for employee training as it relates to persons served plans, goals, support, and behavioral issues.
- Assists persons served with hygiene, physical, and mobility needs as necessary.
- Accurately passes all medications when applicable.
- Promotes and maintains a positive relationship between the program and its persons served, families, staff, volunteers, and other agencies.
- Completes and maintains all required trainings.
- Performs other incidental and related duties as required and assigned.
Management / Supervisory Responsibilities :
Assume a leadership role in the absence of the Center Manager.
Qualifications :
Education, Experience, Skills :
- Bachelor’s degree in social service or a related field, (QDDP qualified) is preferred.
- Previous experience with individuals with intellectual and / or developmental disabilities is preferred.
- Possess sound oral and written communication skills as well as decision-making skills.
- Possess good listening and comprehension skills along with strong leadership skills.
- Must be able to write and document in a clear, concise, and professional manner.
- Possess the ability to use Microsoft Office applications including, but not limited to Outlook, Word, and Excel.
- Ability to work as a team member.
Certifications, Licenses, Registrations :
Driver’s License
Physical Demands / Working Conditions :
- Drive large van to and from events, programs, etc. Must assist persons served entering and exiting the van. Lock down wheelchairs on vans.
- Physically assist persons served as needed.
- Lift persons served who cannot bear weight.
- Push and maneuver wheelchairs.
- Moderate Work Exerts up to 50lbs of force occasionally and 20lbs frequently. Standing, sitting, reaching, typing, and walking are common.
