Proposed Schedule : 40 hours per week. Monday-Friday 8am-4 : 30pm.
This position has the option of a hybrid work arrangement once trained with up to two days per week being remote.
Position Summary
The Ryan White Program / Infectious Disease RN is responsible for delivering care to patients utilizing the nursing process of assessment, planning, intervention, implementation and evaluation.
Additional responsibilities tracking of quality measures and quality improvement data for the Ryan White Program and providing rapid HIV and Hep C testing in both clinical and non-clinical, community settings.

Essential Duties and Responsibilities :
- Effectively interact with patients and their families ensuring patient flow is maintained, nursing resources are being appropriately utilized and that high-quality nursing care is being provided
- Provides age-appropriate, population-specific nursing care in a compassionate, caring, culturally and ethnically sensitive manner
- Protects patients and employees by adhering to infection control policies and protocols
- Maintains a professional and cooperative relationship among all members of the health care team by communicating information, building rapport, and participating in team problem solving methods
- Maintains a hands-on approach and is clinically competent as a registered nurse
- Prepares the patient during the intake process by reviewing the check- in document, obtaining pain level, vital signs, height / weight, date of last LMP (if indicated), general history and chief complaint
- Reconciles the medication list and prepares Rx. refills for provider review and signature. Reviews allergies, immunization list, social history and quality measures section of the EMR
- Assists in compliance, clinical quality measures, and staff development duties
- Responds to patient phone calls using symptom- based policies and protocols to classify urgency of symptoms and advise intervention options
- Assists with management of clinical inboxes allowing all staff members to work to the highest level of their license / job duties
- Collaborates with staff and other departments in the development of strategies to continuously improve quality of care rendered
- Facilitates communication between providers and patients. Assists with patient flow and coordinates huddles at the start of each session
- Triages walk-in patients and assists with medical emergencies within the center
- Ensures operation of medical equipment by following manufacturer’s instructions, troubleshooting malfunctions, and calling for repairs
- Able to oversee and re-direct LPNs and Medical Assistants in the absence of the nurse manager completing appropriate documentation and communicating concerns with Nurse Manager
- Leads nursing staff in team-based model of patient clinical care.
- Provides guidance and mentors new and incumbent staff based on skill set
- Assist with oversight of standards, competencies, policies and procedures, point of care testing (CLIA), including specimen collection and proper documentation in the electronic medical record
- Demonstration of proper medication management processes, utilization of informatics (EMR), and evidence-based guidelines
- Attends staff meetings and participates on committees as requested
- Provides regular feedback to the Nurse Manager and Ryan White Director
- Actively contributes to the professional development, morale and teamwork of staff while presenting a positive attitude and patient-minded vision, with patient satisfaction as the continuing goal
- Provides early intervention and primary care services to HIV / AIDS patients
- Ensures patient treatment plans are coordinated between the Ryan White Providers and other primary care / specialty providers
- Abstracts patient information and quality measures into the Ryan White data system, monitoring HRSA / HAB quality measures
- Tracks Quality Measure data using specialized Excel spreadsheets, Athena and CAREWare. Assists Program Director with data reporting for the RWP Quality Management Committee, annual plan, HRSA required RSR and HHHN NQRCC.
- Collaborates with Alliance for Positive Health regarding case management services.
- Participates in the program rapid HIV testing initiatives.
Qualifications :
The requirements listed below are representative of the knowledge, skill and ability to perform the essential functions :
- Registered Nurse. Must possess an A.A.S. degree or higher from an accredited school of nursing
- Minimum of one-year clinical experience preferred. Prior acute care or ambulatory care experience preferred
- Must possess an unrestricted license to practice nursing in New York State
- Must have a valid driver’s license and be able to travel throughout the Network
- Must possess current BLS certification
- Demonstrated computer competencies including electronic medical record experience
- Knowledge of current nursing practices and standards of care
- Must be proficient with Microsoft applications
- Must be proficient with the electronic medical record
- Must be clinically competent to serve in a staff nursing role in the Health Center setting
- Must be self-directed, detail oriented, motivated and able to work independently as well as in a team atmosphere
- Must possess excellent verbal and written communication skills in order to interact with colleagues, supervisors, medical staff and other departments
- Must be able to envision the big picture and how the Network’s Nursing Department activities are an extension of other Network initiatives and goals
The pay rate for this position is $36.00 / hour.
