66-$3150.00 semi-monthly Revised : 09.25.2024 Summary Under the direction of the Associate Director, the Permanent Supportive Housing (PSH) Program Manager is responsible for programmatic and contract management associated with the respective properties providing onsite Intensive Case Management Services.

Program oversight includes but is not limited to service provision, crisis management and triage, quality assurance, and coordination with onsite property management and external community partners.

The Program Manager must be able to relate to the Division’s management team and effectively represent the agency in all professional settings.

Essential Functions Provide direct oversight of all programs’ supportive services and activities related to Permanent Supportive Housing Projects that HOPICS is the designated service partner.

Actively participate in strategic planning, implementation, and evaluation processes for PSH. Maintain an in-depth knowledge of LAHSA, Department of Health Services (DHS), and Public Housing Authority policies and procedures, implementing changes as needed.

Develop program policies and procedures and create forms to ensure the effective operation and delivery of high-quality intensive case management services.

Responsible for coordinating with the respective onsite property management to ensure the seamless transition of homeless clients into permanent housing and connection to case management services provided at each respective site.

Ensure access to clinical oversight of case management services is provided and staff interactions with PSH clients are appropriate, monitor program and client service standards.

Recruitment, selection, training, supervision, and evaluation of staff (i.e. performance evaluations) Supervise a staff of Case Managers.

Responsible for development and oversight of a process for client access to case management support and linkage to external resources.

Implement a crisis response system for all PSH clients. Ensure Case Management staff receive appropriate training from funders, HOPICS or other related agencies / organizations that are relevant to housing and homeless services.

Track data related to contractual performance targets, funder reporting, client referrals, case management, caseloads, and service-related trends in HMIS / CHAMP systems.

Cultivate resources and collaborate with community partners and landlords that will enhance service planning and delivery.

Develop field-based and on-site strategies for safety and service provision for clients / tenants at respective PSH sites.

Monitor ongoing service delivery and client satisfaction. Ensure appropriate staff are hired and retained per the funder requirements.

Provide and / or facilitate access to clinical support and guidance to the case management team via consistent individual / group supervision.

Follow a HR approved progressive disciplinary process as needed for all staff. Attend regularly scheduled and as-needed coordination and / or team meetings with funders, Property Management and HOPICS.

Maintain appropriate boundaries with clients and staff and adhere to SSG’s Code of Ethics and HOPICS’ Core Values Represent the SSG / HOPICS in a professional manner at meetings (in-person and virtually) and at community events.

Facilitate the Quality Assurance process for PSH and ensure all data quality is accurate and timely. Maintain files / records and prepare project reports on client services in compliance with HIPAA & CFR42 and other funding requirements for auditing purposes.

Prepare and submit monthly reports, invoicing and contract compliance benchmarks. Manage and provide HMIS and / or CHAMP data entry goals and benchmarks statistics.

Troubleshoot with HOPICS management team to develop strategies to remove barriers to achieving program / monthly goals and outcomes.

Facilitate collection of program data related to productivity, contract performance, outcomes, demographics and integration efforts.

Must know, understand, and be able to articulate the mission, vision, and core values of HOPICS. Ability to critically think, understand and carry out oral and written direction Participate in related community special events Reinforce and maintain division policies and procedures Provide division-wide leadership and management in alignment with HOPICS core values.

Coordinate and work in conjunction with other HOPICS program directors and managers. Prepare and update check requests, subcontractor agreements and other required documents Attend all related trainings and meetings Regular attendance required Other duties as assigned Minimum Qualifications - Knowledge, Skills and Abilities Required Bachelor’s degree in human services, social work or a related field;

and 3 years or more experience in social service field. In depth knowledge of the Los Angeles County / SPA 6 homeless services landscape.

California Driver License, Proof of Car Insurance, Reliable transportation. If in recovery, a minimum of three (3) years of being drug and alcohol free is MANDATORY Ability to work with clients and staff from diverse cultural, ethnic, and socio-economic backgrounds.

Ability to communicate effectively, both written and orally Ability to work with homeless clients with chronic physical and behavioral impairments.

Ability to organize and manage multiple priorities. Experienced in Microsoft Office Suite. Non-traditional or weekend work may be required.

Familiar with Homeless Management Information System (HMIS) and CHAMP (Client Track) System preferred. Verification of Employment Eligibility and Background Check.

Bilingual abilities, highly desired. Supervisory Responsibilities The Program Manager will supervise a team of PSH Case Managers at various sites.

Relate to other people beyond giving and receiving instructions Effectively influence people on a consistent basis Must be able to problem solve without immediate supervision as appropriate Accept and carry out responsibility for direction, control, and planning Physical Requirements While performing this job, the incumbent typically spends time sitting, standing, walking, speaking, thinking, listening, and carrying (20lbs).

Must sit at the computer for data entry and reports : sometimes spend time driving and walking and carrying equipment / files / charts.

Speak to staff inside and outside agencies in public meetings and Provider Meetings. Mental Requirements This position will require the individual to be able to handle any / all the following : constant distractions, interruptions, uncontrollable changes in priorities / work schedules : be able to process information, think and conceptualize.
