MAIN FUNCTION :
Provide nursing care to residents age 16 and over who require Skilled Long Term and short Term Care including the chronically ill, geriatric and infirmed resident.
The primary purpose of your job position is to render routine daily nursing and restorative care and services in accordance with the resident’s assessment and care plan and under the direction of a professional nurse.

Reports to the Nursing Supervisor or Unit Director.
DUTIES AND RESPONSIBILITIES :
PERSONNEL FUNCTIONS :
- Follows work assignments, and / or work schedules in completing or performing tasks.
- Performs all tasks in accordance with established policy and procedure and as instructed by the
Professional Nurse.
Receives the nursing report upon reporting in and gives reports to Charge Nurse before ending
shift duty hours.
- Wears photo identification card at all times.
- Records all entries on flow sheets, nutrition sheets, elimination record, activities record, rehabilitation record and completes all documentation in electronic form, or as instructed.
- Reports all changes in the resident’s condition to the RN or LPN Charge Nurse as soon as practical.
- Reports all accidents and incidents observed on the shift as they occur, providing statement for same.
- Participates in performance assessment and improvement activities for continued quality
improvement of resident care.
NURSING CARE FUNCTIONS :
- Performs routine resident care which includes :
- Bathing
- Mouth Care
- Hair Care
- Nail Care
- Hygiene
- Toileting Assistance
- Feeding
- Ambulation Assistance
- Promotes resident comfort to include :
- Positioning, lifting and transfers
- Ambulation
- Answering call lights
- Assisting with emotional needs
- Observes and reports presence of pressure areas and skin breakdowns to prevent decubitus ulcers.
- Assists with post mortem care.
- Takes and records weights, temperature, pulse, respiration and blood pressure as directed.
- Answers all call bell / lights promptly.
- Promotes adequate resident nutrition by participation with meal service and feeding residents.
- Serves and assists residents with in-between snacks and nourishments as assigned.
- Collects and transports specimens as directed.
- Assists in the care of the patient in isolation.
- Performs prescribed rehabilitation nursing exercises, range of motion and use of adaptive equipment.
- Folds and delivers resident’s personal clothing.
CARE PLAN AND ASSESSMENT FUNCTIONS :
- Assists with carrying out the Resident Assessment.
- Participates in Care Plan conferences to develop and review Resident Care Plans.
- Provides nursing care according to established Care Plan.
- Recommends changes in the resident’s Care Plan and goals based on observations and
assessments. Reviews residents closet door care plan, informs nurse of any changes in patient condition.
SAFETY AND SANITATION :
- Operates all equipment in a safe manner after receiving inservice on how to use equipment.
- Reports defective equipment to Unit Director / Charge Nurse immediately, labels same and informs Unit Clerk to order repair request.
- Is responsible for cleaning resident equipment such as commode chairs, wheelchairs, shower chairs, and geri-chairs.
- Follows policies and procedures related to Standard Precautions.
- Keeps resident’s rooms, dresser drawers, closet, utility rooms, shower / tub rooms and corridors neat and in order.
- Follows New York State Requirements for PPD testing.
- Follows emergency protocols related to Fire / Disaster procedures.
STAFF DEVELOPMENT :
Attends continuing educational programs designed to enhance knowledge of the care of residents
requiring Skilled Nursing as well as meet nurse aide certification requirements.
- Participates in DNV and OSHA mandated inservices annually.
- Promotes and fosters the mission and philosophy of the Facility during performance of job duties and community activities.
- Actively supports and participates in Continuous Quality Improvement endeavors.
- Performs other related duties as required.
RESIDENT’S RIGHTS FUNCTIONS :
- Maintains confidentiality of all residents care information.
- Maintains the rights of all residents to independent expression, decision, personal freedom, dignity, and freedom from abuse or social isolation.
- Provides privacy for residents / knocks before entering the resident’s room, closes door or curtain before performing procedures.
- Reports resident’s complaints or grievances to Charge Nurse as soon as possible.
OTHER :
- Is responsible for attending all annual mandatory educational programs as required by position.

EDUCATION :
High school graduate preferred.
EXPERIENCE :
Previous experience or job knowledge is required. Nursing aide certification is required. If hired without certification, must complete the requirements for nurse aide certification for job retention within 120 days.
CARDIOPULMONARY RESUSCITATION (CPR) REQUIREMENTS :
Heartsaver CPR required within 90 days of hire date. Basic Life Support (BLS) required within 90 days of hire date.
PHYSICAL DEMANDS :
Good physical and mental health is required because long periods of standing and walking are involved.
Also, stretching and lifting are required in the care of patients, the equipment and the physical environment.
- Manual dexterity is necessary for handling instruments and equipment.
- Visual and aural acuity are required to make adequate observations of resident’s condition.
SPECIAL REQUIREMENTS :

- Trustworthy in safeguarding confidential information.
- Willingness to work hours that best meet the needs of a specific service.
- Appearance to be neat and professional at all times.
- This position requires limited access to the medical record, which includes charting on the flow sheet,
ADL and graphic.
OTHER REQUIREMENTS :
The certified nurse aide is listed as current in the New York State Nurse Aide Registry, that the CNA’s current nurse aide certification status has been verified with the NYS Nurse Aide Registry, that there are no findings or convictions of resident abuse, mistreatment, neglect, or misappropriation of resident property listed on the Registry and the CNA, and that the printed verification from the Registry verifying all the above is filed in the CNA’s personnel records.
