Seeking a kind and passionate, part-time RN to work in consumer’s homes to provide care for them. Responsibilities include monitoring vital signs, providing basic medical treatment, and consulting consumer’s families.

Minimum Qualifications :

• High School Diploma / GED
• 18 years of age or older
• Completed vocational nursing program

Current state license as a registered nurse and BLS certification required

• Ability to speak, read and write in English
• Ability to follow oral and written instructions
• Excellent interpersonal skills
• Ability to work flexible hours, including evenings, weekends and some holidays
• Valid driver’s license, reliable vehicle and current automotive insurance preferred
• Ability to effectively communicate with consumer, families and doctors

Experience in home health care

Essential Responsibilities :

• Arrives to work on time as scheduled
• Attends mandatory company meetings
• Accurately follows instructions provided by management team

RN Responsibilities :

• Complies with the company’s rules and state and federal laws regarding consumer care. Performs duties in accordance with each consumer’s Individual Service Plan (ISP).
• An ISP outlines the areas in which the consumer needs support and / or training from a personal attendant. An ISP covers various topics, including but not limited to;

self-advocacy, safety, health improvement and maintenance, recreation, hygiene, meal planning and cooking, shopping, public transportation, menu planning, money management including banking, medical and dental and other community resources

• Checks and monitors vital signs, like heart rate, blood pressure or blood sugar
• Ensure the consumers take their medication
• Keep updated medical records including log of blood sugar
• Send monthly reports to doctor
• Communicate results to doctor, verbal and / or written
• Collect samples to transport to lab
• Monitor, report and record symptoms or changes in consumer’s behavior and / or condition
• Prepare medication box weekly
• Medication pickup, as needed
• Schedules and attends (if needed) medical appointments and arranges transportation
• Regularly communicates with consumer’s family, conservator, and interdisciplinary team

Monitor LVNs responsibilities

• Perform duties in the utmost ethical manner by, maintaining consumer confidentiality
• Ability to manage high stress situations, including responding to consumer’s injuries or illnesses
• Ability to work on a team to support consumer
• Willingness to receive feedback regarding, and to improve upon work performance
• Takes initiative without prompting from supervisors

Physical / Mental Requirements

• Bending, crouching, stooping, stretching, kneeling, and reaching or similar activities
• Lifting objects up to approximately 60 pounds
• Occasional lifting or transferring persons
• Sustained alertness and concentration required to supervise consumers
• Ability to hear and communicate with consumers
• Ability to negotiate with consumers
