Job Description :
- Responsible for case review and telephone communications with patients for assessment, and assistance with appropriate level of care needs and / or safety planning if appropriate.
- Responsible for answering phone calls, telephone assessment and triage, and scheduling full patient assessments with appropriate level of care resource.
- Functions as a member of the multi-disciplinary treatment team.
- Responsible for communications across entire health system related to patient referrals and outcomes.
Qualifications :
Bachelor’s degree in Social Work, Psychology, Counseling, or related field required.
Two years of experience in primary discipline preferred.
Wisconsin Social Work or RN license preferred.
