We invite you to join our leadership team as the Director of Social Services. In this highly visible role, you will report directly to the Nursing Home Administrator.
The primary purpose of your job position is to plan, organize, develop, and direct the overall operation of our facility’s Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Administrator, to assure that the emotional and social needs of the resident are met / maintained on an individual basis.
Essential Duties :
- Direct, establish & plan the overall policies / goals for the Social Services Department
- Assist in the development, administering, and coordinating of department policies and procedures.
- Stay up to date on current federal and state regulations, as well as professional standards, and make recommendations on changes in policies and procedures to the Administrator.
- Review department policies and procedures at least annually, and participate in making recommended changes
- When necessary, create required procedures
- Responsible for assessments, care planning & helping residents adjust
- Ensure appropriate discharge planning
- Ensure all residents have advanced directives in place
- Complete Admissions, Psychosocial Assessments and all other necessary assessments
- Occasional Community Outreach
Education, Experience and / or Training :
- LMSW preferred
- LPN / RN license a plus
- Bi-lingual fluency in English and Spanish preferred
- Long-term Care experience required
- 2-4 years management experience.
- Comprehensive knowledge of City, State and Federal regulations pertaining to licensed skilled nursing facilities.
