As a Bilingual Clinical Program Navigator, you will assist patients in the linkage and retention of care within the Southwest Center Wellness Clinic.
In partnership with community resources and programs, you will be actively engaging with patients who present a need for PrEP, nPEP, HIV Management, STI Treatment, Family Planning, Reproductive Care, or Gender-Affirming Care and Resources.
We are looking for a person who is committed to removing the patient’s barriers to care by helping them navigate through health care systems and services while promoting medical and social wellness.
This person will report to the Clinical Program Manager (Title X + PrEP).
We are seeking a candidate that can take action and think critically in unique circumstances to ensure that the patient's care is a top priority.
This person will have a neutral approach to all aspects of HIV care, prevention, and gender affirming services. The ideal candidate will show initiative, demonstrate confidence, and have the ability to provide a wide range of services.
This person will be open to continuous learning, have strong organization skills, and be able to execute tasks that are required to successfully manage their caseload independently.
The Clinical Program Navigator supports the Southwest Center mission of providing affirming and inclusive services to promote well-being and advance health equity for diverse communities and all those seeking compassionate care - especially people of color, LGBTQIA2S+ and Queer individuals, and those affected by HIV.
What your day will look like at Southwest Center :
Conduct one-on-one education sessions and assessments with patients, to determine specific healthcare needs and barriers, to include referrals for internal and external services (e.
g., pre-exposure prophylaxis (PrEP), post-exposure prophylaxis (PEP), treatment, and general wellness / primary care.)
- Take responsibility for an active caseload of at least 60 80 individuals that require assistance in Title X, Gender-Affirming Care, or Prevention service lines.
- Demonstrate culturally appropriate and culturally reactive engagements with all individuals within the community, performing patient intakes, tracking progress, documenting records, and referrals, as well as communicating with patients via telephone, email, and in-person, completing all required documentation within 72 hours of the encounter.
- Introduce biomedical prevention services (PrEP / nPEP / Treatment as Prevention), gender-affirming care, and general wellness visits;
ensuring access, follow up, and adherence to medication and healthcare visits, including supportive services that can enhance harm reduction, behavior change, and maintenance.
Eliminate barriers to care through insurance navigation and enrolling patients in qualifying patient assistance programs that increase access to care.
Insurance navigation includes assisting patients in the legal processes of enrolling in State Medicaid (AHCCCS).
- Offer guidance and assistance for legal name changes and gender marker changes; assist in obtaining the required supplemental documentation to do so.
- Aid with appointment management at SWC, as well as collaborate with internal staff members to ensure consistent quality care, advocating for patients and community members alike.
- Provide information and referrals for other partnering agencies that provide additional support and services to include surgical and medical needs that cannot be met by SWC.
- Provide clear, understandable, current, and accurate information to clients, community members, and volunteers.
- Provide prevention and harm reduction information in a way that shows respect, active listening skills, empathy, and patience in a non-judgmental, stigma-free way.
- Collaboratively work with all Clinical Programs to further develop and evolve the needs of the department, including but not limited to, policy reformation, training new-hires, and identifying opportunities of growth and improvement within the agency.
- Research, adapt, and restructure resources and referrals that specifically relate needs to the transgender and / or non-binary community, as well as the queer community.
- Work directly with the department managers and leads with other prevention-related tasks.

Requirements :
Who we're looking for :
- You've graduated from High School or completed your GED
- You are proficient in Microsoft Office (Word, Excel, Outlook, PowerPoint) software applications and you are competent in using electronic medical records and information systems in providing effective documentation of patient care.
- You have a full understanding of HIPAA and how to protect patient confidentiality.
- You demonstrate a sound understanding of gender identity, gender expression, gender presentation, sex, and sexual orientation etc.
- You have a strong grasp of local and national resources (medical, mental health, social etc.) for those that are transgender or gender non-conforming.
- You demonstrate an understanding of basic medical terminology.
- You are a highly organized self-starter with excellent problem-solving skills and possess the ability to multi-task effectively and you are willing to learn.
- You demonstrate excellent written and verbal communication.
- You have the ability to work in fast paced environment with changing deadlines and priorities while maintaining a positive attitude.
- You have demonstrated cultural competency to professionally and respectfully serve the LGBTQIA2S+ communities.

- You are bilingual in both English and Spanish

- You have personal or professional experience regarding members of the transgender and / or non-binary community, as well as all components of the LGBTQIA2S+ community.
- You possess a valid driver’s license, insurable driving record, and access to a reliable personal vehicle for work-related activities.
- You have electronic Medical Record experience.
Environmental Factors, Physical Requirements, and Compliance
- Work primarily in a climate-controlled environment with minimal safety / health hazard potential.
- Office environment, exposure to computer screens for lengthy periods of time.
- This is a non-exempt position that may require evening and occasional weekend work.
- While performing the duties of this job, the employee is regularly required to walk, sit, stand; use hands, talk, and hear.
The employee is occasionally required to reach with hands and arms. Specific vision abilities required by this job include close vision, distance vision, and ability to adjust focus.
- Currently have, or be able to obtain within 90 days of employment, a valid Fingerprint Clearance Card
- Currently have, or are able to obtain within 7 days of employment, a clear TB test
- Currently have, or are able to obtain within 7 days of employment, a current flu vaccination
- Currently have, or are able to initiate within 7 days of employment, a Hepatitis-B primary vaccination series
- Currently have, or are able to obtain within 14 days of employment, a CPR certification
