Project Homes mission is to end long-term homelessness and positively impact the lives of those who we serve. Project Home serves single adults and families in 5 counties : Steele, Dodge, Waseca, Rice, and Freeborn.
The Project Home Case Manager works to connect, coordinate, and monitor community resources that will empower the participants to function independently.

Duties / Responsibilities :
Under supervision of the Program Coordinator, this position completes supportive housing case assessments, alcohol and chemical dependency assessments, develops appropriate community-based care plans to stabilize housing and maintain single adults and families safely and effectively in the community within Agency standards of practice.
Provides case management services to adults and families, coordinates services provided by outside agencies, provides ongoing assessment of services to clients ensuring they are appropriate and effective, provides ongoing assessment of local care systems to assure needed resources are available, maintains client case records, completes activities and records to maximize revenue reimbursements, and participates in professional development activities.
Required Knowledge, Skills, and Abilities :
Knowledge on mental health and mental health treatment services preferred.
Knowledge on homelessness and supportive housing preferred.
Ability to effectively connect and work with participants while maintaining appropriate and healthy boundaries.
Ability to schedule ones times and activities efficiently while also prioritizing duties.
Ability to maintain patience, confidence, and composure under pressure or adverse circumstances.
Education and Experience :
Meets ONE of the standards for Mental Health Practitioner per DHS guidelines as listed below :
oHave at least 30 Semester hours or 45 quarter hours in behavioral health or related fields from an accredited college or university and have at least 2000 hours of supervised experience in the delivery of services to persons with mental illness, traumatic brain injuries (TBIs), developmental disabilities, and / or substance use disorder OR
oHave at least 4,000 hours of supervised experience in the delivery area of services with adults with mental illness, traumatic brain injuries (TBIs), developmental disabilities, and / or substance use disorder OR
oHas a master's or other graduate degree in behavioral sciences or related fields.
Physical Requirements :
On occasion assisting participants move will be requested if staff have the physical capabilities to perform the tasks.
Work Environment :
Office is located on the 2nd floor of a building without an elevator.
Applicants must be able to tolerate and adapt to a daily schedule that is subject to change due to participant needs, rescheduled, or cancelled appointments, and participants that do not show up for appointments.
Some duties are performed within an office setting while most duties with participants occur in the participants home or in community settings.
Potential ability to work both in the office and from home once fully trained.
