  • Job Responsibilities
  • Responsible for the coordination and facilitation of the plan of care.
  • Assessment of complete chart and knowledge of patient's medical condition, financial status, emotional needsand support systems.
  • Collaborates with the Medical Director, Director, Manager, and Supervisor of Case Management on patientswho are not meeting the regulatory requirements for hospitalization.
  • Provides initial and concurrent reviews for approval of hospital stay for all patients. Notifies patients whenstay has been denied.
  • Collaborates with the interdisciplanary team to ensure patient quality of care and safety of discharge.
  • Provides education to physicians, staff and patients / families.
  • Education
  • Associate's degree in Nursing or Diploma in Nursing required. Bachelor's degree in Nursing preferred.
  • Licensure / Certification / Registration
  • Licensed as a Registered Nurse (RN) required. Eligible to practice nursing in the State of Kentucky required.Certified as a Basic Life Support (BLS) Healthcare Provider preferred.
  • Work Experience
  • 2 to 8 years progressive experience in a related field required. Experience in Case Management preferred.
  • Skills and Attributes
  • Requires critical thinking skills and decisive judgment. Works under minimal supervision. Must be able to workin a stressful environment and take appropriate action.
