Responsibilities
Under the Direct Supervision of the VP of Physician Services, the Patient Navigator will work in collaboration and continuous partnership with patients and their family / caregiver(s), clinic / hospital / specialty providers and staff, and community resources to connect patients to relevant community resources, with the goal of enhancing patient health, well-being, decreasing patient readmissions and increasing patient satisfaction.
Qualifications
MINIMUM EDUCATION :
High School Graduate or GED Equivalent; relevant experience in lieu of High School Diploma / GED education will be considered.
MINIMUM WORK EXPERIENCE :
3 years previous customer service experience.
REQUIRED LICENSES / CERTIFICATIONS :
None
REQUIRED SKILLS, KNOWLEDGE, AND ABILITIES :
Previous experience in a customer support role. Strong phone and verbal communication skills along with active listening.
Customer focus and adaptability to different personality types. Ability to multi-task,priorities, and manage time effectively.
Ability to establish and maintain strong working relationships with staff and management; excellent organizational skills with emphasis on detail and ability to effectivelycommunicate both verbally and in writing.
Detailed oriented, team player, and an independent thinker.
