PRSC Psychiatric Rehabilitation Services Coordinator

Hickory Village Nursing & Rehab, a skilled-psych facility located in Hickory Hills, Illinois currently has a part time PRSC.
Position Summary : To provide social service needs to adults in a long term care / nursing home setting.
This position requires
BA / BS, MA / MS or above in Social Work or related field preferred.
Flexible hours working weekend rotations
Must be able to work well within the context of a Long-Term Care facility.
Demonstrates good judgment under stress, emotional stability, tact, reliability and ability to adapt changing circumstances
Other responsibilities include :
Providing the resident with a stable therapeutic relationship
Orienting the resident to the facility.
Reviewing and assisting the resident in understanding his / her treatment plan and program schedule i.e. day program, in-house groups and activities.
Preparing and assisting the resident with active participation in his / her treatment plan review.
Providing and / or coordinating the delivery of the psychiatric rehabilitation services programs
Monitoring the resident in the areas of self-directed care and for overall compliance with their treatment plan.
Completing proper documentation upon admission, quarterly, and annually.
Documenting the resident’s progress on a regular basis, including incidental, monthly and quarterly notes.
Contacting community agencies for the purpose of resident referral.
Maintaining knowledge of residents on psychotropic medications, including the identified purpose of a specific medication and participating in behavioral programming to help the resident achieve his / her highest functional level.
Lending group and individual counseling sessions, as assigned.
Being familiar with the facility policies on behavioral emergencies and be trained in crisis management and diffusing a potentially volatile situation, especially concerning aggressive / agitated behavior.
Participating in educational training's and in-services provided by the facility.
Monitoring smoke breaks.
All other duties as assigned.
