Job Overview :
The Community Based Services Provider ( CBSP ) provides a range of behavioral health support services to youth within our Home and Community Based Services ( HCBS ) program.
Youth in this program have identified, or are at risk of, mental health, substance abuse, or developmental challenges. All services are intended to improve their health and well-being and to maintain them in their homes and communities.
Services are delivered in the youth’s home, school, or other community settings, and are always goal-directed based on the individual needs of that youth and family.
The CBSP staff person will primarily deliver Caregiver / Family Advocacy Support Services (CFASS), Pre-Vocational Services and Respite.
These services, under the HCBS umbrella is designed to restore, rehabilitate, and support a child’s / youth’s developmentally appropriate functioning as necessary for the integration of the child / youth as an active and productive member of their family and community.
Responsibilities include, but are not limited to :
- Engage directly with youth and families to Provide 1-1 teaching, coaching, and rehabilitative counseling to build the skills and capacities of a youth struggling with mental health and developmental challenges.
- Engage with youth in their home and in the community to ensure for a person-centered approach.
- Provide services as identified on the youth’s Treatment / Service Plan following the specific frequency, scope, and duration.
- Assist families to better understand and parent their child with mental health and developmental needs.
- Attend all mandated training as assigned by their supervisor.
- Regular and ongoing communication with other providers and family to advocate for a youth’s needs, mental health wellbeing and any non-healthcare needs (ex.
housing, employment, social support).
- Timely and accurately document the delivery of services in an electronic health record.
- Local travel required to provide the services in the youth home, school, or other community settings.
- Maintain a positive, strength-based and person-centered approach at all times.
- Provide other support and rehabilitative services as the agency determines, based upon new contracts or changing regulations.
- Additional position-related duties and responsibilities as assigned by the supervisor.
Position Requirements :
- Bachelor's Degree in social work, psychology, or in related human services field PLUS two (2) years of applicable experience in children’s mental health, addiction, foster care / child welfare / juvenile justice, and / or a related human services field.
- Must have a clean and valid New York State Driver’s License, be actively licensed for at least one year and, AND have a reliable vehicle for transporting clients - with proof of 100 / 300 liability insurance coverage.
- Ability to safely transport young children, which may include lifting and carrying a young infant and / or toddler.
- Ability to work flexible hours to accommodate client schedules which may include some early evening hours.
- Must possess outstanding interpersonal and communication skills, with a strong attention to detail and excellent writing aptitude.
- Ability to pass required pre-employment background checks, to include fingerprinting.
- This position does have regular and substantial unsupervised or unrestricted contact with individuals receiving services and is required by law to be fingerprinted for a criminal history record check under DOH (NYS Department of Health).
