The Program Coordinator is responsible for the coordination of the Center for Key Populations (CKP) Mobile Health Units (MHU).

The Program Coordinator is the program's key liaison between patients, communities who receive services from the MHU and administrators;

assists the program supervisor / manager and directors in developing and / or completing reports, processes and policies related to the Mobile Health Unit Program.

ROLE AND RESPONSIBILITIES

  • Develop policies, procedures, manuals, and training as needed
  • Ensure compliance with all safety and regulatory requirements for DPH license.
  • Complete and monitor data entry, record keeping, and reporting for staff and patients
  • Conduct and maintain community outreach and collaboration with community organizations and partnerships
  • Work with MHU staff to coordinate schedule of hours and services
  • Manage client enrollment and participation
  • Coordinate equipment transports with CHC Facilities department
  • Establish and maintain a community resource directory
  • Act as the CHCI safety officer for the MHU program
  • Assist with the facilitation of regular meetings with staff and CKP leadership to ensure effective communication
  • Assist to train staff for the MHU program
  • Maintain comprehensive records for staff training and vehicle inspection
  • Monitor MHU 1&2 vehicle maintenance, safety updates, cleaning, sanitation and all other physical attributes of vehicles
  • Plan and implement community events to promote services of MHU Program
  • Performs other related duties as assigned

QUALITFICATIONS AND PREFERRED SKILLS

  • Associate degree required, BA / BS in Human Services or related field preferred
  • Prior experience working with community agencies and programs
  • Experience with Microsoft Excel, Word, and Outlook
  • Strong oral and written skills are required.
  • Demonstrates ability to work cooperatively with providers and agencies
  • Must have excellent attention to detail
  • Prior experience in providing services to bicultural individuals / families desired
  • Ability to organize, prioritize, and maintain deadlines
  • Working knowledge of the program, its target populations and additional resources available in the community
  • CHC requires as a condition of employment current American Red Cross CPR for the Professional Rescuer and AED (CPR / FPR / AED) certification.

The only acceptable alternative is current American Heart Association BLS / AED for Healthcare Providers certification

PHYSICAL REQUIREMENTS / WORK ENVIRONMENT

This position requires some physical exertion, mostly in support of groups in a program area.

Work is both remote, office-based and community-based

WORK SCHEDULE DEMANDS

Full-Time, 40 hours a week with evenings and / or weekends required based on program needs. Ability to travel to locations as deemed necessary
