Position Summary

This remote role requires a HIPAA-compliant workspace for work at home. Residence in or within an approximate one hour or less distance from New Albany, OH (Columbus, OH area) is required for travel to an office for a quarterly and any additional scheduled on-site meetings as needed.

Pharmacist may have overnight travel (less than 10%) if a plan sponsor requests pharmacist attendance at an onsite meeting not held in New Albany, OH.

Usual working hours are 8 : 00 am-5 : 00 pm or 9 : 00 am-6 : 00 pm in the time zone of residence. There are no weekends or holidays expected with this role.

A Pharmacy Program Manager is a Clinical Pharmacist providing pharmacist support for an interdisciplinary care management team and members under assigned plan sponsors through conducting comprehensive medication reviews, providing drug information, making phone outreach to members and providers, identifying and resolving medication related interventions.

This role does not involve managing staff. A Pharmacy Program Manager carries responsibilities associated with supporting members under assigned plan sponsors and providing front line services.

This is not a people management role.

Pharmacy Program Manager role includes :

  • Collaborates with an interdisciplinary care management team (nurse case managers, dietitians, social workers, medical directors) to support members on their path to better health, decrease hospital readmissions, and reduce medical costs.
  • Provides comprehensive medication reviews, drug information, and medication education.
  • Supports members under assigned plan sponsors and the care management team acting as a subject matter expert."
  • Participates in case conferences, grand rounds, on-site meetings, and clinical assessments.
  • Triages medication related issues to appropriate areas / disciplines to obtain answers / resolution, interprets formulary, and reviews high cost medications for assistance opportunities.
  • Transitions daily between completing independent work and collaborating with a team while having the ability to prioritize urgent needs and multi-task.
  • Develops and maintains working relationships with care management team as well as internal and external departments.
  • Aids in the implementation strategy of workflows and processes to monitor for effectiveness.
  • Strong interpersonal and communication skills are essential to communicate with appropriate audience (i.e., members, providers, colleagues, etc.).

Required Qualifications

  • Active Ohio pharmacist license in good standing
  • 6+ years of clinical pharmacy experience required
  • Current or past employment in a hospital and / or ambulatory care setting (excluding pharmacy school clinical rotations / internship)
  • Currently on a daily basis applies clinical knowledge to review medications and resolve different disease management scenarios beyond a dispensing role;

identifies medication related interventions while resolving and offering appropriate treatment options to provider.

Demonstrated ability to review and apply clinical literature / pharmacy resources to provide clinical reviews / inquiries / newsletters.

Preferred Qualifications

  • Managed care experience
  • Advanced training (i.e., Board-certified pharmacist, residency, fellowship, completion of APhA certificate programs)
  • Medication Therapy Management (MTM) experience and / or certificate
  • Strong computer skills (MS Office, Microsoft Teams, claims databases, proprietary software)
  • Strong oral / written communication, interpersonal skills, and presentation skills within a matrix organization to solve problems and achieve team / company goals.
  • Ability to prioritize and multi-task multiple referrals simultaneously.
  • Ability to adjust communication style to suit the audience (patient, colleague, providers, business management etc.).
  • Ability to work both independently and as part of a team.

Pay Range

The typical pay range for this role is :

$87,035.00 - $187,500.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.

The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.

This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
