JOB SUMMARY :
An individual employed to assist in the activities of the Referral Specialists and office staff of Commonheart Home Health.
This involves the performance of a variety of clerical, scheduling, and administrative duties. This position is accountable to the Director of Home Health.
ESSENTIAL JOB FUNCTIONS :
- Maintains and actively promotes effective communication with all individuals.
- Maintains a positive image of the entity in the community keeping in alignment with our mission, vision, and values.
- Responsible for on-call services, as assigned.
- Establishes and maintains positive working relationships with current and potential referral sources.
- Maintains patient charts and assists with referrals.
- Maintains the confidentiality of patient and agency information at all times.
- Manages the daily nursing schedule to ensure alignment with productivity standards.
- Implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy.
- Gathers, collates, and reports referral statistics including key customer referral trends.
- Performs analysis of admission and discharge trends by business unit in order to accurately plan referral capacity.
- Performs other specific projects relating to intake, data entry, and computer operations as required.
- Manages the initiation of service and continuous monitoring of telehealth units
- Abide by all Commonheart Home Health policies and procedures.

EXPERIENCE AND TRAINING :
- High School diploma or equivalent required.
- Education or certification in office administration preferred.
- 2-3 years’ experience performing administrative tasks (scheduling, phone calls, customer service, etc.) required. Experience in a health care or human services setting preferred.
- Must be knowledgeable in the general use of the computer and other office equipment.
