Description : JOB SUMMARY :

JOB SUMMARY :

An individual employed to assist in the activities of the Referral Specialists and office staff of Commonheart Home Health.

This involves the performance of a variety of clerical, scheduling, and administrative duties. This position is accountable to the Director of Home Health.

ESSENTIAL JOB FUNCTIONS :

  • Maintains and actively promotes effective communication with all individuals.
  • Maintains a positive image of the entity in the community keeping in alignment with our mission, vision, and values.
  • Responsible for on-call services, as assigned.
  • Establishes and maintains positive working relationships with current and potential referral sources.
  • Maintains patient charts and assists with referrals.
  • Maintains the confidentiality of patient and agency information at all times.
  • Manages the daily nursing schedule to ensure alignment with productivity standards.
  • Implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy.
  • Gathers, collates, and reports referral statistics including key customer referral trends.
  • Performs analysis of admission and discharge trends by business unit in order to accurately plan referral capacity.
  • Performs other specific projects relating to intake, data entry, and computer operations as required.
  • Manages the initiation of service and continuous monitoring of telehealth units
  • Abide by all Commonheart Home Health policies and procedures.

EXPERIENCE AND TRAINING :

  • High School diploma or equivalent required.
  • Education or certification in office administration preferred.
  • 2-3 years’ experience performing administrative tasks (scheduling, phone calls, customer service, etc.) required. Experience in a health care or human services setting preferred.
  • Must be knowledgeable in the general use of the computer and other office equipment.
